Remove Driver word from the statement

The problem is that we need to remove the word 'driver' from the statement. Some customers have W-2 drivers while others have 1099 (independent contractor) drivers, and including the word 'driver' could cause issues when a statement is sent to a 1099 contractor, or we have to create a new type of statement for 1099 contractor

There is a driver ID in statement written when we choose 12 type and it must be removed from there also we have to add logo of the company there.
Driver#: (id of the driver) - Name of the driver
